Care Coordination

We believe in making a lasting difference is by following the 3 steps of our Care Coordination program.

1.

Crisis Intervention/Prevention: Connecting with participants through different avenues to get them connected to a plan for stabilization.

2.

Crisis Stabilization: Building and maintaining a care plan with a participant, identifying goals, implementing actionable steps and connecting to other service providers so that the participant is able to stabilize and grow. 

3.

Care Advocacy: Community members are empowered to volunteer by serving participants through the stabilization process. 

CITYSERVE

Care Session

CityServe offers Care Sessions for individuals that have more than just a single basic need, where they meet with one of our Care Coordinators to create a Care Plan based on their current situation.  This Care Plan consists of Goals and Action Steps that are tracked for better outcomes.  Helping us be confident in our caring,  and create better relationships with our Service Partners and clients.

CityServe CARES about your story and wants to hear your needs.  So the first step is to schedule a Care Session with a Care Coordinator.

During your Care Session the Care Coordinator will build an individualized goal focused CARE PLAN based on your needs.

CityServe CONNECTS you to vetted local resources within the community tracking the outcome of each referral making sure that you are getting the care needed.

Care Wheel

Everyone has a story and it doesn’t matter if you need help in one category or all of them, we are here to help you find the resources you need!

Our team of care coordinators uses a model we call the “care wheel” to assess and individuals basic needs, emotional health and well-being, community, connections, physical health, vocational/educational and financial health. CityServe then educates, resources and refers participants to specific services, but we also ensure participants are given the tools, documents, and information needed to access those services. Our unique approach is infused with compassion and allows CityServe to meet a wide variety of needs in collaboration with our Mercy Space Partners.
