SSPTV was established in 1981 with a Friendly Visitor volunteer program serving 35 seniors. It has since grown to include programs which annually serve approximately 1,600 seniors in the Tri-Valley area.

Our mission is to provide services and assistance to seniors that will foster independence, promote safety and well-being, preserve dignity, and improve quality of life.  We pride ourselves in listening to the needs of our area’s senior citizens and their families to create programs to fit those needs. Since 2006 we have been an independent non-profit 501(c)3 organization. Funding for our programs is provided through grants from the cities of Pleasanton, Livermore, and Dublin, the Alameda County Departments of Behavioral Health, Area Agency on Aging, and Public Health, and Measure A Alameda County Health Care Services, Supervisor Scott Haggerty, Public Authority for In-Home Support Services, Alameda County Transportation Commission, Kaiser Permanente, and Individual Donations.


Programs offered:

Case Management- Individualized needs assessment with the coordination of services and resource management for older adults

Family Caregiver Support Program – Monthly support groups to provide resource information and assistance with life transitions

Friendly Visiting- Connects volunteers 1:1 to isolated seniors living in the community for companionship
VAST Transportation- Volunteer drivers provide eligible seniors a ride to essential medical appointments
In-Home Registry-Links seniors who are looking for more support in the home to care providers in the community
Finding Wellness Program- Series of classes promoting health, wellness, and safety
Fall Prevention-A variety of programs to help reduce the risk of accidental falls. Exercise/A Matter of Balance/ Tai Chi
Medication Safety-an evidence-based medication review to improve medication adherence
Healthy Lifestyles Program-Individualized nutrition consults and nutrition education
Health Services-Foot care and various health screenings by appointment provided at senior community sites
In-Home Counseling -Short term, home-based therapy by professionals

Volunteering Information (Duties / Roles Needed Friendly Visiting Program: Volunteers will visit isolated seniors in the Tri-valley, who are typically home-bound, for socialization and companionship. Volunteers may accompany the senior to the grocery store or choose to grab a cup of coffee together. Volunteers can make both home visits and/or phone call visitations. A 6 month commitment would be much appreciated. VAST Transportation Program: Volunteers will drive eligible seniors to essential medical appointments both in and outside of the Tri-Valley area. A 6 month commitment would be much appreciated.
