Care Coordination (Case Management) Services

Care Coordination supports the individual needs of Tri-Valley seniors to maintain an independent life style.

A care coordinator will assess a senior’s ability to perform Activities of Daily Living (ADL’s).  This information will help determine the level of care assistance an individual may need. After the assessment, an initial care plan is created and the care coordinator assists the senior to meet the goals of the plan.

Care plans often address the following issues:

  • Transportation
  • Meals
  • Housing
  • Shopping
  • Medical appointments
  • Emergency response plans/Devices
  • In-home services and support
  • Volunteer support services
  • Elder abuse prevention/intervention
  • Problem solving
  • Referrals (as needed)
  • Applying for Medi-Cal/SSI
  • Social integration


The care coordinator will monitor the care plan and also match with a volunteer care advocate for ongoing support specific to the needs of the senior.

Funding is provided by the city of Pleasanton, Alameda County Area Agency on Aging, and Community Development Grants through the cities of Dublin and Livermore.
