Unsheltered (homeless) & At-Risk Population (Full-Time)
The Position: We’re looking for a part/full-time (20 to 40 hours a week) Case Manager/s– Unsheltered Population. The pay range we’re offering is $22-26/hour. Typical hours are Monday through Friday, 8am to 5pm, but position may require some evenings & weekend scheduling as assigned.

Responsibilities: This Case Manager will do extensive outreach to identify vulnerable individuals/families and provide housing/shelter navigation, benefit enrollment, assist with obtaining vital documents and caring case management services to unsheltered individuals or those at imminent risk of experiencing homelessness in and around the Cities of Livermore, Dublin, and/or Pleasanton. Case loads vary on client need level and position hours.
- Provide case management services to unsheltered individuals. Case management includes meeting with individuals to verify and understand: (1) the need for housing; (2) status of housing history; (3) identification of any existing housing alternatives; (4) assessment of client’s ability to sustain housing, e.g. issues of mental health, stability and amount of income, employment readiness if unemployed or underemployed, and credit history; (5) individual’s and families’ financial assets.
- Will be responsible for providing a participate-centered, strengths-based case management approach to assist families and individuals to a state of economic or other independence, as defined as “significant and sustainable life change” based on the outcomes prioritized by CityServe Services Team and our funding streams.
- Excellent data keeping, including confidential personal files, case notes, etc in multiple databases, including Apricot Social Solutions and HMIS. Responsible for providing content for monthly reports due to funding streams.
- Responsible for seeking out resources, government agencies, and other non-profits on behalf of unsheltered individuals to pave the way for the most efficient communication with and for the client, as needed. Collaboration with organizations such as (but not limited to) Abode Services, Inc., Echo Housing, Bay Area Community Services, the Livermore Housing Authority, and the affordable housing departments and police departments in the Cities of Livermore, Dublin and/or Pleasanton will be especially important.
- Required: A Bachelor’s degree in a Behavioral Science, Social Science, or a closely related field; OR
- A Bachelor’s degree in any field and one year of human services related experience (i.e. work providing assistance to individuals and groups with issues such as inadequate housing, economically disadvantaged, employment, abuse and neglect, substance abuse, aging disabilities, prevention, health); OR
- An Associate’s degree in a Behavioral Science, Social Science or closely related field and one year of human services related experience (see item #2 above); OR
- Three years of human services related experience (see item #2 above)
- Bilingual (English/Spanish) preferred.

- Effective communication skills and ability to multi-task, including ability to answer phones and ability to community sensitively, efficiently, effectively and appropriately with a diverse set of clients, and with co-workers and managers.
- Experience working in a social/human services field interviewing clients to obtain personal, sensitive information and determine eligibility for programs/benefits and the ability to do so with patience and genuine interest.
- Experience communicating decisions, rules, regulations, policies and procedures to clients or customers verbally and in writing in a respectful, client-centered way.
- Experience with case work and assessing clients’ strengths and service needs that is person centered and trauma informed.
- Experience creating and maintaining written documentation of the activities concerning individual client cases.
- Experience working with community partners to assist clients in becoming self-sufficiency.
- Experience with complex work systems and possessing strong prioritization skills to organize and manage a variety of tasks.
- Strong computer skills and experience to enter/retrieve and manage data. The ability to use different software for data entry, case notes and eligibility determination.
- Ability to accurately use basic mathematics in a work environment.
- Demonstrate effective trauma-informed conflict resolution and de-escalation skills with clients, co-workers, and manager.
- Must be punctual, have a good attendance record, and have reliable means of transportation to work.
The Location: The main office is based in Pleasanton, CA. This position’s area of service will be Tri-Valley wide (Livermore, Pleasanton, Dublin).
